Job Title: Occupational Therapist Profession: Occupational Therapy Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours, M-F, flexible Experience: 1+ years of experience in Occupational Therapy License: Active Occupational Therapy license required Certifications: None specified Must-Have: - LTC or Rehab Experience - New Grads considered with Level II experience Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress and treatment plans. Provides comprehensive discharge summary of services per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Participates in facility and company required trainings, in-services, and conferences. Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements. Able to supervise and support occupational therapist assistants and rehabilitation aides. Adheres to state required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
