Make a meaningful difference as a travel Occupational Therapist in Phoenix, AZ. Provide coordinated, patient-centered care in a home health environment. Plan, implement, and evaluate individualized plans of care to help patients develop, regain, or maintain daily living skills. Evaluate occupational roles and performance for patients with physical, psycho-social, and cognitive impairments. Teach skills, techniques, and adaptive equipment use. Collaborate with the interdisciplinary team and document patient activities, progress, and orders in a timely manner.


Minimum Qualifications

  • Graduate of an accredited occupational therapy program with a Bachelor of Science, Master’s, or Doctoral degree in Occupational Therapy.
  • Current state license as an Occupational Therapist in Arizona is required to start the assignment.
  • Proof of current BLS/CPR certification required.
  • Current valid driver’s license, reliable transportation, and active vehicle insurance required.
  • Minimum two years of Occupational Therapist experience in acute care or rehabilitation settings required.
  • Ability to perform clinical assessments, develop and revise Plans of Care, and document progress notes and phone orders.
  • Experience with fabrication and maintenance of adaptive equipment and hand splints.
  • Ability to work a flexible schedule, travel locally, and perform physical duties including lifting, bending, pushing, and prolonged standing/walking.


Preferred Qualifications

  • Prior home health experience preferred.
  • Experience in interdisciplinary collaboration and working with fiscal intermediaries or third-party payers preferred.


Assignment Details

  • Shift name: Days.
  • Workweek: 5 shifts per week, 8-hour shifts.
  • Ability to float or travel locally as needed.
