Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Therapy Duration: 13 weeks Shift: 5x8.0 Hours per Shift: Monday - Friday 8:00 AM - 4:30 PM; weekends as needed Experience: Minimum of one year of experience in a Long Term Care/Home Care setting License: Valid OT license to practice in Client Must-Have: OASIS Description: Provides case management and direct patient care within a team environment. Administers comprehensive skilled care. Participates and assures that care delivery services align with organizational vision, mission, values, and strategic goals while meeting customer needs. Promotes effective working relationships and works effectively as part of the team to meet the goals and objectives. Promotes a professional image by adhering to the dress code policy. Utilizes resources, supplies, and time efficiently to contain costs and enhance service value. Promotes positive, supportive, respectful communication to all external sources, patients, families, and staff. Consistently displays behaviors in support of the overall organization. Supports public relations and marketing initiatives within the community. Strives for continuous improvement in all aspects of the Occupational Therapist function and care delivery process using quality management principles. Develops and revises treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payers. Provides an environment that respects patients’ privacy, confidentiality, and property. Initiates appropriate preventive and rehabilitative strategies. Consults with and educates patients, families, and other team members regarding disease processes, self-care techniques, and preventive strategies. Provides supervision to ensure patient health, safety, and compliance with plans of care. Directs, coordinates, evaluates, and supervises the quality of patient care services provided, including clinical supervision of aides. Interacts with patients, physicians, referral sources, and others in a manner conducive to maintaining positive relationships. Displays an exemplary level of patience, courtesy, and tact. Supports collaborative and customer-focused planning and case management designed to meet individual health needs and promote quality and cost-effective outcomes. Participates in regularly scheduled patient conferences with team members to discuss patient care and ensure a comprehensive, coordinated plan of treatment. Ensures completion of all necessary clinical records for compliance with legislations and the delivery of seamless service within established timeframes. Refers to appropriate community agencies and resources via the management system. Maintains current knowledge of available community-based services. Demonstrates flexibility, enthusiasm, and cooperation while working with customers and others whose support is necessary. Ensures problem resolution at the point of contact. Makes constructive recommendations regarding quality of patient care and operational improvements. Sets an example of integrity. Assists leadership in monitoring and collaborating with customers to identify potential new programs and services. Ensures patient needs are met through the care delivery process and responds to customer needs appropriately. Ensures authorizations and release of information forms are signed by patients or responsible parties. Participates in achieving operational and financial objectives.
