Job Opportunity – Certified Occupational Therapy Assistant (COTA) – Portland, OR 97225

Start Date: August 24, 2026

Location: West Portland, OR and surrounding areas

Schedule: Day shift, 5x8-hour shifts (8:00 AM - 4:30 PM), Monday through Friday. Up to 6 patient visits per day with 30 productivity points standard. One-on-one patient care in home health setting. No on-call or weekend requirements.

Highlights:

  • Manageable caseload with efficient client grouping to minimize drive time
  • EPIC Remote Client for Home Health and Rover app charting system
  • Business casual or scrubs allowed; free parking provided
  • 91-day assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)
  • Mileage reimbursement at IRS standard rate
Requirements:
  • Active Oregon COTA license required
  • NBCOT certification and current BLS certification required
  • Minimum 1 year of skilled home health experience required
  • Reliable personal transportation required (occasional travel up to 60-mile radius)
  • COVID-19 primary vaccination series plus one booster required
  • Experience with OASIS documentation preferred; HomeCare HomeBase charting experience preferred
  • Two professional references covering 1 year within the last 3 years
