Premier Medical Staffing Services is seeking a travel Occupational Therapist for a travel job in Fort Dodge, Iowa.
Job Description & Requirements

HIGHLIGHTS:
- 2 years of previous OT experience [required] - home health experience [preferred] but is NOT necessary!
- Epic experience [preferred]
- State license [required]
- Must have own vehicle - Mileage will be reimbursed
ADDITIONAL DETAILS:
- Avg. patients per day: 4-6
- Patient population: Most patients need short term therapy after hospitalization or nursing home stay (varied reasons for original facility admission)
- 1 PTA and 1 COTA on staff along with 1 PT and 1 OT
- Coverage area: 50 miles of Fort Dodge, IA.
- Weekly team meetings occur virtually; monthly in-person meetings
- Orientation: First day general organization orientation and required online courses. 2 days of Epic (EMR) training, couple days doing ride along/shadow visits.
- Typically fully productive by week 3.

- Driver s License [required]

- Skills checklist (must have been completed within 1 year of submission date) [required]
- License verification [required]
- Copy of required certifications
- References - 1 supervisory reference required at time of submission [required]
- **Must have taxable address outside of 50 miles from facility. **
MORE INFORMATION:
- As a member of the interdisciplinary team, the traveler contributes professional/clinical knowledge and skills in the provision and management of care to patients.
- EMR:EPIC
- Call Back: 2 hr CB minimum
- Summer Holidays: Traveler will be required to work 1-2 of 3 (Memorial Day, Independence Day, Labor Day)
- Submission requirements: Not an employee of UnityPoint in the last 12 months [required]
