Job Summary:
Occupational Therapist (OT) Home Health in California. This position involves planning and conducting segments of treatment programs aimed at restoring and improving patients' physical, social, and mental functions within a home health setting. The role includes serving as a co-leader in an interdisciplinary approach to patient care, ensuring personalized treatment plans, and maintaining comprehensive clinical documentation.

- Required Qualifications: Master's degree in Occupational Therapy (OTD) obtained license on or after January 2007, or foreign equivalent; valid Occupational Therapy licensure in all applicable states of practice; CPR or BLS certification from approved vendor per organization policy; valid Driver's License (DL) and minimum age of 21 years.
- Essential Functions: Completes evaluations ensuring quality standards, establishes individualized treatment plans, provides therapy interventions, communicates discharge and care plans, completes clinical documentation within deadlines, supervises assigned assistants, performs other related duties.
- Additional Information: Home health experience is preferable but not required. All associates must receive required vaccinations as a condition of employment, with exemptions available for medical or religious reasons.
