Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Occupational Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current Occupational Therapy license from the state of practice.
• BLS certification required.
• Arizona Fingerprint Clearance Card required at time of hire and must be maintained for the duration of employment.

Assignment Details

• Shift hours: 0800-0500.
