Job Summary

We are seeking a specialized Wound Care RN to join the NYC Health + Hospitals team. In this role, you will lead health assessments and physical examinations focused on wound, ostomy, and continence care. You will be responsible for developing individualized plans of care, recommending treatments, and evaluating patient responses to complex wound therapies. This position requires a clinical expert who can collaborate across disciplines and provide essential education to both patients and staff.

Key Responsibilities
  • Clinical Assessment: Conduct focused physical assessments and health interviews for patients with wounds, ostomies, fistulas, and continence issues.
  • Care Planning: Develop, implement, and revise standards of care and individualized treatment plans based on prudent clinical assessment.
  • Prevention: Routinely assess patients at risk for pressure injuries and initiate preventative measures.
  • Education: Develop and evaluate staff education programs (orientation, in-services) and counsel families on prevention and home care.
  • Quality Improvement: Participate in hospital-wide Q.I. activities, including incidence and prevalence studies and nursing quality audits.
  • Asset Management: Oversee rented equipment including specialty beds and Negative Pressure Wound Therapy (NPWT) devices.
  • Collaboration: Work with the Assistant Director of Nursing for Wound Care to recommend best practices and escalate ethical or conflict-related challenges.
Logistics & Environment
  • Location: Lincoln Hospital, Bronx, NY.
  • Cultural Competence: Must provide care consistent with diverse cultural, ethnic, and religious belief systems.
  • Confidentiality: Strict adherence to data security and HIPAA/patient rights policies.
  • Community: Identify and refer patients to available community resources to support long-term recovery.
