Must-Have: - MED/SURG NURSING - WOUND CARE - COLOSTOMY CARE - PATIENT ASSESSMENT - CARE PLANNING Description: Job Summary: Conduct health assessment interviews focused on wound, ostomy, and continence-related care. Perform focused physical assessments and develop plans of care based on diagnoses and individual client needs. Recommend medications and treatments as appropriate. Job Responsibilities: Develop, revise, and evaluate standards of care. Assist in the selection of supplies, technology, and equipment. Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments. Evaluate the quality and delivery of care provided by clinical staff. Work collaboratively with the healthcare team and make referrals to other disciplines as indicated. Teach, counsel, and assist individuals and families to assume responsibility for prevention of complications related to ostomies, wounds, or continence problems. Develop, provide, and evaluate staff education programs, including new employee orientation and continuing education. Routinely provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures. Escalate and communicate challenges identified, including ethical issues and conflict resolution. Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices based on prudent assessments. Participate in quality improvement activities and studies related to nursing quality. Provide culturally competent patient, family, and staff education consistent with various belief systems. Maintain the security and confidentiality of data and information in accordance with patient rights policies. Achieve and maintain professional competency through engagement in clinical practice. Identify available community resources helpful to clients and assist them in utilizing such resources. Manage assets related to rented equipment, such as specialty beds and negative pressure devices.
