Job Title: Wound Care Nurse Profession: Nursing Specialty: RN Duration: 14 weeks Shift: Days Hours per Shift: 5x8 Experience: Three to five (3-5) years of experience in healthcare. License: Active Registered Nurse license. Certifications: - Certified Wound Ostomy Nurse (CWON) - Certified Wound Care Nurse (CWCN) - Certified Wound Specialist (CWS) - Wound Care Certified Certification (WCC) Must-Have: Wound care certificate Description: Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy, and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients. Conduct health assessments focused on wound, ostomy, and continence related care. Perform focused physical assessment and develop a plan of care based on the diagnosis and individual client needs. Recommend treatments as appropriate. Assist with the development, revisions, and evaluations of standards of care. Assess and document patient response to wound care, ostomy, continence, fistula, and complex wound treatment. Evaluate the quality and delivery of care provided by the clinical staff. Work collaboratively with the healthcare team and make referrals to other disciplines as indicated. Teach, counsel, and assist individuals and families to assume responsibility for the prevention of complications related to patients with ostomies, wounds, or continence problems. Provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures. Participate in Unit/Department Quality Improvement and hospital-wide quality improvement activities. Provide culturally competent patient, family, and staff education consistent with cultural, ethnic, religious, and other belief systems. Maintain the security and confidentiality of data and information in accordance with patient rights policies. Perform other tasks as assigned.
