Responsibilities
- Perform comprehensive wound assessments for acute, chronic, surgical, diabetic, pressure, and vascular wounds
- Develop, implement, and evaluate individualized wound care treatment plans
- Perform advanced wound care procedures, including dressing changes, negative pressure wound therapy (NPWT), debridement assistance, and ostomy care as appropriate
- Monitor wound healing progress and adjust care plans based on patient response
- Collaborate with physicians, surgeons, dietitians, physical therapists, case managers, and interdisciplinary healthcare teams to optimize patient outcomes
- Educate patients, families, and caregivers on wound prevention, treatment, nutrition, and home care management
- Document assessments, interventions, wound measurements, and patient progress accurately in the electronic medical record (EMR)
- Assist with pressure injury prevention initiatives and hospital quality improvement programs
- Ensure compliance with infection prevention standards, patient safety guidelines, and evidence-based wound care practices
- Provide consultation and clinical support to nursing staff regarding wound management best practices
Requirements
- Active California Registered Nurse (RN) License
- Minimum 2 years of recent Wound Care nursing experience
- Current BLS Certification required
- Wound Care Certification (WCC, CWCN, CWON equivalent
- Strong knowledge of wound assessment, treatment modalities, pressure injury prevention, and ostomy care
- Experience with negative pressure wound therapy (wound vacs) preferred
- Proficiency with electronic medical record (EMR) systems
- Excellent assessment, communication, documentation, and patient education skills
- Ability to work independently while collaborating effectively with multidisciplinary healthcare teams
- Commitment to delivering evidence-based, patient-centered wound care
