Job Title: Transplant Coordinator Profession: Registered Nurse Specialty: Transplant Coordinator Duration: 13 weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 16:30) Experience: Minimum of 3 Years License: Valid Nursing License Certifications: BLS, Chemo (ONS/ONCC) Must-Have: Hematology/Oncology experience Navigator/Case Manager experience Description: RTO after booking is not allowed and cannot be worked out onsite. Any call-offs or additional RTO on unit may lead to a cancellation. Hematology/Oncology experience is a must. Navigator/Case Manager experience is required. Clinical assessment skills include interpretation of lab results and diagnostic tests related to transplant. Thorough medical history of potential and actual donors is essential. Must monitor vital signs, weight, fluid balance, and graft function. Review and interpret relevant diagnostic and laboratory data. Coordinate all patient care needs before, during, and after transplantation. Adherence to patient confidentiality and HIPAA regulations is required. Provide patient and family education as needed. Schedule and track pre- and post-transplant testing. Foster collaboration within the transplant team and adhere to transplant center protocols. Understand post-transplant health maintenance and long-term care. Know the waitlist criteria applicable to the transplant process. Specialties will include peripheral blood transplant cellular therapy. Candidates follow through various phases of patient care, from identification through evaluation, cell harvest, and transplant. Organizational skills and the ability to work autonomously are essential. Daily interactions may involve various parties on behalf of the patient. No call or weekend coverage is required. Coordination of medication adjustments post-transplant with the provider is necessary. Recognition and management of transplant complications such as infection or rejection must be demonstrated. This role demands familiarity with organ-specific diseases such as ESRD, cirrhosis, and heart failure. Effective communication and teaching of medication adherence and postoperative care plans are important. Documentation must track patient outcomes per regulatory standards. Assistance with organ offers and acceptance is also part of the responsibilities. No RTO will be allowed during the first week. All requests for time off must be pre-approved. Candidates must be open to floating within and near the facility as required by the position. References may be pending at submission.
