Position Summary

St. Joseph Hospital is seeking a compassionate and skilled Registered Nurse (RN) to join its Rehabilitation Unit. The RN will provide high-quality patient-centered care, support patient recovery and rehabilitation goals, collaborate with interdisciplinary teams, and ensure safe, effective care delivery. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and a commitment to quality outcomes.

Key Responsibilities
  • Provide professional nursing care to rehabilitation patients in accordance with physician orders and established nursing standards.
  • Develop, implement, and update individualized patient care plans each shift.
  • Perform patient assessments, monitor progress, and communicate changes in condition to the healthcare team.
  • Deliver bedside shift reports and participate in multidisciplinary rounds to ensure continuity of care.
  • Educate patients and families regarding treatment plans, recovery goals, medications, and discharge planning.
  • Maintain accurate and timely clinical documentation within the electronic medical record.
  • Collaborate with physicians, therapists, case managers, and other healthcare professionals to optimize patient outcomes.
  • Participate in hourly rounding and patient safety initiatives.
  • Assist in identifying and resolving patient care and safety concerns.
  • Delegate appropriate patient care activities to nursing assistants and support staff.
  • Serve as a preceptor and mentor for new nursing staff when assigned.
  • Participate in quality improvement initiatives and regulatory readiness activities.
  • Promote a positive, patient-focused, and team-oriented work environment.
  • Support infection prevention and patient safety standards.
