Job Title: Wound and Ostomy RN Profession: Nursing Specialty: Wound Care Duration: 13 weeks Shift: Days Hours per Shift: 5x8 Experience: Minimum of 2 years preferred License: Registered Nurse License Certifications: BCLS Must-Have: Wound Care Certificate Description: The Wound and Ostomy RN is responsible for providing direct and indirect patient care in inpatient care areas. The RN is expected to competently perform patient care responsibilities within the assigned work setting. Responsibilities include the systemic collection and assessment of data in collaboration with the multidisciplinary team to provide therapeutic care for the patient and/or family. The RN performs all required elements of a nursing assessment, functional assessment, and utilization of patient/family-specific assessment as required. The RN considers factors related to safety, effectiveness, and cost in planning and delivering care. Decisions and actions on behalf of patients are determined in an ethical manner. Care is provided in collaboration with other nursing staff members; the RN delegates and supervises care in accordance with nurse practice acts and the professional nursing role. The RN demonstrates expertise in wound and ostomy nursing and the nursing process through the provision of nursing care. Direct care in the hospital is provided. The RN systematically assesses the needs of the patient through interview, observation, and physical examination. Information about the medical plan of care and previous health care experience is secured when applicable. Progress in meeting defined outcomes on the multidisciplinary plan of care is noted. Individual nursing care is planned and organized for an identified group of patients. The RN administers individual nursing care, independently applying both simple and complex techniques and processes specific to the needs of the patient. The nursing care administered to patients is evaluated by the RN. Care is delegated in accordance with Nurse Practice Acts and care guidelines. Patient responses to prescribed interventions are documented and evaluated. Patient and family teaching is completed, including providing information about unit policies, procedures, and provisions for self-care and participation in decision-making. Patient and family participation in care is elicited in keeping with physiological and psychological readiness. Patients and/or families are instructed on subjects such as disease processes, risk factors, and prevention of complications. Anticipation of potential nursing and/or medical problems from present evidence is practiced, and preventive measures are initiated. Nutritional status of patients is assessed and communicated with the healthcare team. Patient and family progress in meeting defined outcomes is reported to physicians and members of other health care disciplines. The RN skillfully uses a range of equipment, medications, and other modalities pertinent to the assigned work area. Concise communication regarding patient progress is provided to nursing staff members and other healthcare practitioners. The principles of growth and development for appropriate age groups are understood. The ability to assess data reflective of the patient’s status in relation to appropriate age groups is demonstrated. Relevant information needed to identify each patient’s nursing care requirements relative to age-specific needs is interpreted. Nursing care relative to the patient’s age-specific needs is provided. The Wound and Ostomy RN establishes satisfactory interpersonal relationships with co-workers and colleagues, patients, families, and significant others. Communication with patients and families is initiated to determine individual needs. Communication with co-workers and colleagues in other disciplines and departments is initiated to share and exchange ideas and information. Detailed patient education is provided to patients and family members during the inpatient stay and at discharge. Effective responses to communications from patients, families, subordinates, colleagues, and supervisors are maintained. Decisions and actions on behalf of patients are conducted in an ethical manner. Sensitivity to the values of self and others is demonstrated. Patient and family involvement in the plan of care, incorporating cultural, spiritual, and belief systems, is ensured. Physical demands involve standing, walking, sitting, talking, hearing, and bending. The position may include some repetitive motions. This position involves performing and assisting with assessments, interventions, and evaluations for patients with actual or potential wound/ostomy problems. Activities include dressing changes for patients, assisting with Vacuum Assisted Closure (VAC) devices, implementing plans of care created by the Wound and Ostomy RN, and assisting in the management of pressure injuries and other wound/skin problems. The role also includes assisting in quality improvement and clinician education activities related to wounds and skin care. Main duties include providing professional nursing care to patients. The RN systematically collects and assesses data in collaboration with the multidisciplinary team to provide therapeutic, evidence-based care for patients and families.
