Employs independent judgment to provide comprehensive patient care. Collects relevant data pertinent to the patient’s health or situation. Analyzes assessment data to determine diagnosis and care issues. Develops plans that prescribe interventions to achieve patient outcomes. Implements care plans and coordinates care delivery effectively. Evaluates progress toward attaining established outcomes. Identifies specific outcomes for patients or patient situations. Collaborates with patients, families, and healthcare providers in a safe and caring environment. Provides learning opportunities for patients and family members. Delivers health information to patients, families, and treatment teams effectively. Participates in discharge planning to ensure continuity of care.
