Registered Nurse – Oncology (Transplant Coordinator)

Key Responsibilities- Serve as a Transplant Coordinator managing a caseload of patients through all phases of peripheral blood transplant and cellular therapy, from candidate identification through workup, evaluation, cell harvest, and infusion
- Coordinate all patient care needs before, during, and after transplantation, including scheduling and tracking pre- and post-transplant testing according to required timelines
- Interpret laboratory results and diagnostic data relevant to transplant; monitor vital signs, weight, fluid balance, and graft function
- Attend interdisciplinary team meetings with physicians and staff; collaborate with the transplant team to ensure continuity of care
- Provide patient and family education, including medication adherence, post-operative care plans, and lifestyle adjustments
- Maintain compliance with transplant center protocols, HIPAA/patient confidentiality standards, and universal precautions
Required Qualifications- Minimum 3 years of nursing experience required
- Hematology/Oncology experience required
- Navigator and/or Case Manager experience required
- Experience in both acute hospital and outpatient clinic settings required
- Demonstrated ability to perform pre-transplant evaluation and assessment, including review of medical history and interpretation of relevant diagnostic and laboratory data
- Active RN licensure (NC or Compact) and current BLS certification required
Preferred Experience / Skills- Familiarity with organ-specific diseases (e.g., ESRD, cirrhosis, heart failure) and ability to identify and manage transplant complications such as infection and rejection
- Experience with post-transplant medication management, including reviewing pre-transplant regimens for contraindications and educating patients on self-administration
- Knowledge of organ allocation processes, absolute and relative contraindications to transplant, and regulatory outcome tracking and reporting
- Chemotherapy certification (ONS/ONCC) preferred
