Seek and evaluate information acquired from other members of the interdisciplinary team, including patient, family, physician, and support staff. Collaboratively develop individualized plan of care using assessment information and critical thinking skills. Actively seek patient, family, team, and physician involvement to develop the plan of care, including necessary education. Plan and prepare patients for further support needs once current episode of care outcomes are met, such as teaching, referrals, and follow-up. Direct the interdisciplinary care team through delegation, coordination, and collaboration as appropriate. Frequent interactions with patient care providers, patients, and visitors requiring verbal communication and thorough understanding of spoken information. Manual dexterity needed to manipulate complex and delicate equipment with precision and accuracy. Expected to lift and utilize a full range of movement to transfer patients and bend to retrieve, lift, and carry supplies and equipment. May need to walk and assist with transporting/ambulating patients and distributing supplies and equipment. Expected to navigate crowded and busy rooms and stand in a stationary position for an extended period. For roles requiring driving, expected to drive a vehicle which requires sitting, seeing, and reading signs.
