Job Title: Transplant Coordinator Profession: Registered Nurse Specialty: Oncology Chemo Certified Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 3 years License: Required Certifications: BLS, Chemo (ONS/ONCC) Must-Have: Hematology/Oncology experience is a must. Navigator/Case Manager experience is a must. Description: CROSS POSTING FOR JOB ID# 3322071. RTO after booking is not allowed and cannot be worked out onsite. Any call-offs or additional RTO could possibly turn into a cancel and DNR. Hematology/Oncology experience is required. Navigator/Case Manager experience is required. Minimum of 3 years of experience is required. Traveler experience is welcomed, and first-time travelers will be accepted. Local candidates are accepted with a rate difference. Work setting experience in an acute hospital and outpatient clinic is required. Pre-transplant evaluation and assessment skills are required. Candidates must be skilled in interpreting lab results and diagnostic tests related to transplant. Understanding the medical history of potential and actual donors is essential. Patient and family education skills are required. Scheduling and tracking pre- and post-transplant testing is necessary. Collaboration with the transplant team is essential. Understanding post-transplant health maintenance and long-term care is required. Knowledge of universal precautions and waitlist criteria is important. Familiarity with organ-specific diseases is preferred. Coordination of medication adjustments post-transplant with the provider is necessary. Monitoring post-transplant patient progress is required. Offering psychosocial support and education on lifestyle adjustments to patients and families is essential. Teaching medication adherence and post-operative care plans is necessary. Tracking patient outcomes and reporting per regulatory standards is required. Assisting with the scheduling of the OR is a critical function. Transplant coordinators will have patient cases to follow from candidate identification through various phases of the process. The daily census could be up to 8, but total interactions may reach 20 counting all parties involved. Coordinators must be organized and work autonomously while meeting various appointment schedules. There are no call requirements, float requirements, or weekend requirements. Patient diagnoses will include hematologic malignancies. The role entails attending meetings with doctors and staff throughout the week. Candidates must be willing to float within or near the facility as required.
