Job Title: Registered Nurse Profession: Nursing Specialty: Medical Intensive Care Unit (MICU) Duration: 13 weeks Shift: Night Hours per Shift: 12 hours Experience: Minimum of 1 year recent ICU experience License: Valid Michigan license required Certifications: BLS (AHA), ACLS (AHA), NIH Must-Have: - Concrete ETA and proof of application for MI license - No time off in the first two weeks of assignment Description: Holidays are required. No requests for time off in the first two weeks of the assignment. All requests for time off must be presented prior to the offer. Requests received after the offer must be discussed with the manager on site. 1 year of recent ICU experience is required; first-timers are okay. Patient ratios range from 1:1 to 4. Our unit handles a mix of PCU and ICU patients. PCU and general medicine/transfer status can be in a 4:1 group based on acuity. PCU can also be in a 4:3:1 based on acuity. ICU and PCU can mix with a ratio of 3:1 based on acuity. Ventilator patients are in a 2:1 ratio. Cooling, CRRT, or high acuity will be in a 1:1 group. Weekend requirements include working every other weekend. Staff may float to other ICUs and medical-surgical units as needed due to census flexing. Agency RNs will float in rotation with the unit's RNs to medical-surgical units. Typical nurse-to-patient ratios in medical-surgical areas are approximately 1:5 to 1:7. Patient diagnoses include COPD, encephalopathy, drug overdose, DKA, AKI, respiratory failure, cardiac arrest, heart failure, and atrial fibrillation. Special procedures performed include CRRT, hemodialysis, A-line placement, CVC placement, tracheostomy placement, and chest tube placement. Special equipment used includes cooling machines, Bair Huggers, massive transfusion devices, and CRRT. Required skills include cardiac monitoring and emergency response. Interpretation and management of dysrhythmias and multi-system organ failure are essential. Nurses will manage endocrine and metabolic protocols, including IV insulin. Gastrointestinal care includes management of GI surgery. Nurses will handle genitourinary and renal issues like chronic and acute renal failure using CVVHD/CRRT. IV therapy skills required include arterial line management, blood product administration, central line care, and PICC line management. Medications managed include continuous IV sedation, emergency medications, and procedural sedation administration. Nurses will interpret arterial blood gases and suction for both endotracheal tubes and nasotracheal tubes. Ventilator management will be a daily task, with an average of 3 ventilated patients. The unit's ICU physician will maintain a patient's ICU status if deemed necessary regardless of acuity. Stable patients may still be grouped with a ratio of 3 or 4. Daily census averages around 15 patients. The unit has a total of 20 beds. Staffing includes interdisciplinary support from physical therapy, respiratory services, phlebotomy, and pharmacy. Emergency and rapid response teams are also available. Care includes managing conditions such as ARDS, sepsis, pneumonia, acute GI bleed, strokes, traumas, and other critical illnesses.
