• Schedule: Monday-Friday, 8:00 AM - 4:30 PM
• On-Call / Weekend Rotation: Required
• Travel: Local travel between patient visits within the assigned service area

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 1 year of nursing experience required
• Home health, homecare, or home health nursing experience preferred
• Associate degree required
• Strong communication, organization, and time-management skills
• Ability to work independently in a home-based care environment
• HCHB experience is a plus
• Comfortable with a travel assignment and on-call/weekend rotation

Responsibilities

• Provide skilled nursing care in the home setting
• Complete patient assessments and deliver care according to the plan of care
• Monitor patient status and communicate changes to the care team as needed
• Document visits accurately and in a timely manner
• Support safe, high-quality patient outcomes through professional nursing practice
• Travel to patient homes within the assigned area for scheduled visits
• Participate in on-call and weekend coverage as scheduled
