Facilitates patient discharge through timely referrals and patient education resources. Collaborates with other disciplines as appropriate. Supports unit and hospital goals through participation in educational activities. Demonstrates independent judgment in planning operations and making minor decisions in a complex field. Works directly with patients and/or customers. Engages with internal customers via telephone or face-to-face interaction. Collaborates effectively with other healthcare professionals and staff.
