Swanson Model of Care, the RN is responsible for assessing, developing, implementing, and evaluating the plan of care. The RN emphasizes patient and family comfort, education, and satisfaction. The RN uses educational and professional experiences in the delivery of quality care and mentoring/precepting staff. Job Responsibilities: Assessment: Performs assessments and reassessments per policy or as warranted by patient condition. Demonstrates the ability to make sound independent decisions. Development & Implementation: Initiates, prioritizes, and implements the plan of care. Reviews and revises the plan as indicated. Collaborates with patients, families, and interdisciplinary services. Utilizes resources appropriately to support the needs of patients and families. Evaluation: Monitors and documents responses to clinical interventions. Education: Facilitates, coordinates, provides, and evaluates education specific to patient and family needs. Leadership: Delegates tasks to licensed and non-licensed personnel. Functions as a mentor/preceptor. Demonstrates, encourages, and facilitates teamwork. Identifies and communicates opportunities for improvement. Participates in quality, safety, and service process improvement efforts. Implements practice changes and complies with regulatory standards.
