Reports and is accountable to the Nurse Manager/Director. Responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Participates in identifying and meeting the learning needs of self. Attends education programs based on identified learning needs. Uses patient education materials relevant to the patient population with assistance. Assesses readiness to learn of the patient/family/caregivers. Assesses the developmental level of patients and factors affecting the ability to learn. Identifies expected patient outcomes. Seeks guidance as needed in the revision of the plan of care. Participates in discussions with members of the interdisciplinary team in the evaluation of patient care. Communicates relevant information to promote continuity of care. Is knowledgeable about activities that facilitate intra and interdepartmental collaboration. Participates in the development and achievement of unit goals and performance improvement activities. Effectively uses communication systems. Participates in promoting cost-effective care. Provides feedback to coworkers.
