Pay: $3,335.20 per week

Shifts: Variable

Seven Healthcare are seeking an experienced RN - Hospice - Travel to join their team in Green Bay, WI. This role specializes in hospice care, requiring travel to service various locations within the Northern area of Green Bay, Oshkosh, Manitowoc, Sheboygan, and Grafton. The RN will monitor patient referrals and provide hospice assessments within hospital settings.

Key Responsibilities of the RN - Hospice - Travel
  • Monitor queue and consult with patients referred for hospice from hospitals or SNFs
  • Provide assessments and recommendations for hospice settings within hospitals
  • Admit patients to hospice services in a hospital setting
  • Collaborate and communicate with physicians and liaison team
  • Educate providers on hospice benefits and hospice philosophy

Requirements of the RN - Hospice - Travel
  • BLS Certification
  • Valid Driver's License and Auto Insurance
  • Experience with symptom management and end-of-life care
  • EPIC experience required
  • Minimum of 2 years of hospice experience
