Responsibilities
  • Provide comprehensive nursing care to hospice patients in accordance with individualized care plans
  • Perform patient assessments and monitor physical, emotional, and psychosocial needs
  • Administer medications, treatments, and pain management interventions as prescribed
  • Educate patients and families regarding disease progression, symptom management, and end-of-life care
  • Coordinate care with physicians, social workers, chaplains, therapists, and interdisciplinary team members
  • Develop, implement, and update individualized hospice care plans
  • Maintain accurate and timely patient documentation
  • Provide emotional support and advocacy for patients and families
  • Ensure compliance with hospice regulations, patient safety standards, and agency policies
Requirements
  • Active RN License in California
  • Minimum 2 years of recent Hospice, Home Health, Palliative Care, or related nursing experience
  • Current BLS Certification required
  • Hospice and Palliative Credentialing Center (HPCC) certification preferred
  • Strong assessment, communication, and patient advocacy skills
  • Ability to work independently and manage patient caseloads effectively
  • Compassionate approach to end-of-life care and family support
  • Excellent organizational and documentation abilities
