Responsibilities
  • Provide skilled nursing care to hospice patients in home or facility settings
  • Assess patient conditions and develop individualized care plans
  • Administer medications, pain management, and symptom control treatments
  • Educate patients and families regarding hospice care and disease progression
  • Offer emotional support and compassionate end-of-life care
  • Coordinate care with physicians, social workers, chaplains, and interdisciplinary teams
  • Maintain accurate and timely patient documentation
  • Ensure patient comfort, dignity, and quality of life throughout care
Requirements
  • Active RN License in California
  • Minimum 2 years of Hospice or palliative care nursing experience
  • BLS Certification required
  • Strong clinical assessment and pain management skills
  • Excellent communication and emotional support abilities
  • Ability to work independently and manage patient schedules effectively
