License: Must have a valid nursing license in hand. Certifications: CPR, BLS Must-Have: - Proficient in HomeCare HomeBase (HCHB) and Oasis. - Must have reliable transportation, a current driver's license, and agency-required liability insurance. - Must be willing to travel up to 75 miles from the assigned care center. - Must understand that the clinician will accept every patient on their schedule regardless of distance. - Must complete all documentation within 24 hours of the appointment. - Must acknowledge that there are no guaranteed hours with this contract. Description: The Registered Nurse coordinates and directs the provision of home nursing care according to physician's orders. This care is based on agency policies and procedures and involves the competent application of the nursing process. The nurse is responsible to the Clinical Manager. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses are part of the role. The position requires home visits and attendance at periodic office meetings. Daily patient assignments typically range from 5 to 10 patients. The nurse is expected to achieve weekly productivity points of 30, which equates to approximately 40 hours of work. Types of cases include start of care, wound care, and follow-up visits. Clinicians must be comfortable with travel and longer drive times between patients. All documentation must be timely and complete to ensure proper patient care and billing processes. Declining patients may result in cancellation of the contract. The environment requires proficiency in HCHB and Oasis, with recent hands-on experience. Travelers must be open to on-call rotations as required. Local candidates are accepted at the same rate.
