Description:
Coordinates and directs the provision of home nursing care according to physician's orders.
Follows agency policies and procedures through competent application of the nursing process.
Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses.
Home visits and periodic office meetings are required.
Must have reliable transportation, current driver’s license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients daily, equating to roughly 6-8 points.
No guaranteed hours; if unable to meet financial needs due to low patient census, discuss options for floating to a nearby clinic for hours.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS Standard rate.
All documentation must be completed within 24 hours of the appointment with the patient.
Must accept every patient on the schedule, regardless of location, as declining may lead to cancellation.
The position involves being on call.
Local candidates are accepted at the same rate.
No pending licenses will be considered; candidates must have their license in hand.
Experience in hospice is required.
Must be open to on-call rotations.
