Job Title: Hospice RN Case Manager Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift: Typical schedule is Monday to Friday Hours per Shift: 8-5, with on-call and weekend rotation Experience: Minimum of 2 years in acute care, home care, or hospice experience License: Current RN license in the state of Montana required (BSN preferred) Certifications: Valid driver’s license, appropriate auto insurance, and reliable transportation required Description: The RN Case Manager provides skilled nursing, care, and case management for assigned patients in their residence. The role involves admitting patients onto services and coordinating services to ensure continuity of care. This position facilitates the achievement of quality, clinical, and cost outcomes. It involves negotiating, procuring, and coordinating services and resources needed by the patient or family. The RN Case Manager creates opportunities and systems to enhance outcomes. Utilizing the nursing process of assessment, planning, and implementation of care is essential. This includes patient and family instruction and evaluation through an inter-disciplinary team approach. The position may start as a Visiting RN and transition to Case Manager, with the same clinical responsibilities applying to both. Primary duties include providing skilled nursing and coordinating care in a residential setting based on systematic evaluation authorized by a physician. The RN observes, assesses, evaluates, and plans interventions in relation to patients' physical and psychosocial statuses, including changes in condition and responses to treatments or medications throughout their contact with the patient. Oversight of care provided by Home Health Aides and CNAs is required to evaluate compliance with the plan of care. Ensuring continuity of care through effective communication and information exchange with other disciplines and community health agencies is essential. The RN provides quality care and appropriate symptom management to promote optimal health and quality of life. This includes teaching, counseling, appropriate interventions, and referrals. The position requires maintaining accurate, current, and complete patient records in accordance with Agency standards. Knowledge of current reimbursement rules and regulations is necessary. Participation in team scheduling coordination and on-call rotation is required to meet patient care needs. This includes assessing patient needs via telephone and advising or making home visits as required. The RN will participate in quality improvement activities and attend in-service training as assigned. Additional duties may be assigned by the supervisor.
