Job Title: RN Case Manager Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift: Typical schedule is Monday to Friday Hours per Shift: 8 Experience: Minimum 2 years of recent acute care, home care, or hospice experience License: Current RN license in the State of Montana required Certifications: BSN preferred, valid driver's license, and appropriate auto insurance required Must-Have: Strong clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred Description: The RN Case Manager provides skilled nursing, care, and case management for assigned patients in their place of residence. This role admits patients onto services and coordinates and integrates services to ensure continuity of care. The RN Case Manager facilitates the achievement of quality, clinical, and cost outcomes. This position negotiates, procures, and coordinates services and resources needed by the patient or family. Responsibilities include creating opportunities and systems to enhance outcomes. The RN uses the nursing process of assessment, planning, and implementation of care. Patient and family instruction and evaluation occur through an inter-disciplinary team approach. This position may start as a Visiting RN and transition to Case Manager, with the same clinical responsibilities applying to both roles. The RN observes, assesses, evaluates, and plans interventions in relation to patients' physical and psychosocial statuses. This includes monitoring changes in condition and reactions to treatments or medications. The RN oversees care provided by Home Health Aides and certified nursing assistants to assigned patients. Evaluating compliance with the plan of care is essential. The RN assures continuity of care through effective communication and information exchange with other disciplines. Participation in case conferences with community health, support, and social agencies is required. The RN provides quality care and appropriate symptom management for positive clinical outcomes. Promoting optimal health and quality of life through teaching, counseling, appropriate interventions, and referrals is key. The RN maintains accurate, current, and complete patient records in accordance with agency standards. Knowledge of current reimbursement rules and regulations is necessary. Participation in team scheduling coordination and on-call rotation is expected to meet patient care needs. Assessing and evaluating patient needs via telephone and making home visits as required is part of the role. Participation in quality improvement activities and attendance in-service training as assigned is important. The RN performs other duties as assigned by the supervisor.
