Conduct intake assessments and interviews with patients and families. Administer prescribed medications and monitor patient tolerance. Facilitate goal setting with patients and families. Provide education and support on advanced directives and cultural awareness. Manage symptoms such as nausea, vomiting, and constipation/diarrhea. Perform postmortem care as needed. Demonstrate skills in positioning, preventing pressure ulcers, and proper medication disposal. Take part in care coordination with interdisciplinary teams as needed. Maintain documentation in the designated charting system. Ensure compliance with state and federal regulations related to hospice care.
