Job Title: Hospice Intake RN Profession: Registered Nurse Specialty: Hospice Duration: 13 weeks Shift: Day 4x10-Hour Hours per Shift: 08:00 - 18:30 Experience: Minimum 3 years of experience in home care or hospice required License: RN Licensure for the appropriate state Certifications: BLS Must-Have: Minimum 3 years of experience in home care or hospice Description: RTO after booking is not allowed and cannot be worked out onsite. Any call-offs or additional RTO on the unit could possibly result in a cancelation and DNR. Traveler experience: Unit will accept first-time travelers. Locals accepted with a rate difference. Work setting skills required: Home health/hospice and skilled nursing. Professional knowledge and skills required: Intake assessment/interview and patient/family education and support. Must have experience in setting up DME, medications, and supplies for new patients. Strong writing, organizational, and analytical skills are essential. Inpatient hospice experience and a strong understanding of hospice admission LCD criteria are highly preferred. Cultural awareness in end-of-life care is a valuable skill. Daily ratios include 2-3 admissions per day depending on a 10-12 hour schedule. Call requirement: None. Float requirement: Yes, between field and other locations. Weekend requirement: None. Position involves completing the initial clinical assessment. You will review clinical history and current diagnoses. Present findings in a clinical summary. Assess immediate care needs of admitted patients and their loved ones. Initiate the plan of care and send associated orders to the attending provider. Clinical assessments are done through evaluations and interviews. Consult with physicians for appropriate orders. Assist in obtaining needed supplies, equipment, medications, and services. Evaluate hospitalized patients against regulatory criteria and facilitate discharge planning. Enter clear, concise, and timely clinical documentation related to the patient's initial admission and assessment. Must be flexible to travel as needed between different locations. Must be able to operate a motor vehicle in all weather conditions. The unit system used is Epic. Candidates must be comfortable floating based on job descriptions. No restrictions or special requests will be accepted. Travel pairs are allowed but must work on different units. Two manager/supervisor references from within the past 12 months are preferred. Travelers cannot work more than 5 days in a row and/or more than 60 hours in a week. Modules must be completed pre-start and annually, varying by specialty.
