SPECIAL PROCEDURES / UNIT DETAILS: *Completes the initial clinical assessment, reviews clinical history, current diagnoses and presents a clinical summary to the Hospice Medical Director for hospice.*Assesses immediate care needs of the admitted patient and their loved ones, initiates the plan of care and sends assocaited orders to the Hospice Attending provider. Include status and intent in the submission. Local candidates ( 50 miles) : Lower rate applies. RTO : Up to 7 days auto approved (no holidays). Must work 2 of 3 major holidays.
