SPECIAL PROCEDURES / UNIT DETAILS: *Completes the initial clinical assessment, reviews clinical history, current diagnoses and presents a clinical summary to the Hospice Medical Director for hospice.*Assesses immediate care needs of the admitted patient and their loved ones, initiates the plan of care and sends assocaited orders to the Hospice Attending provider.
