Description

We are currently seeking an experienced PACE Admissions Registered Nurse (RN) for a 3-month contract assignment in San Diego, CA. The ideal candidate will have experience in home health or community-based nursing and be comfortable conducting in-home assessments, completing Level of Care (LOC) evaluations, and coordinating care for the frail elderly population.

Position Details:

Specialty: PACE Admissions RN
Location: San Diego, CA
Contract Length: 3 Months (With high possibility of extension)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Guaranteed Hours: Full-Time (40 Hours Per Week)
Weekend Requirement: None
On-Call Requirement: None
Travel: Required for Home Site Visits

Requirements:

  • Active California RN License required
  • Min 1 year experience as an RN
  • Home Health or Community Health experience preferred
  • Experience conducting comprehensive nursing assessments
  • Ability to complete Level of Care (LOC) evaluations
  • Valid driver's license and reliable transportation
  • Experience coordinating home care services and durable medical equipment (DME) preferred
  • Strong documentation and care coordination skills

Skills & Qualifications:

  • Experience performing in-home patient assessments
  • Strong clinical assessment and critical thinking skills
  • Knowledge of care planning and case management
  • Ability to work independently in the community setting
  • Excellent communication and patient/family education skills
  • Strong organizational and time management abilities
  • Experience working with the geriatric or frail elderly population preferred
