We are seeking a compassionate Home Health Registered Nurse (RN) to provide skilled nursing care and case management to patients in their homes. The RN will coordinate care, perform comprehensive patient assessments, develop individualized care plans, educate patients and families, and collaborate with an interdisciplinary team to promote quality outcomes and continuity of care.
Responsibilities- Provide skilled nursing care and case management for patients in their homes.
- Complete patient admissions, assessments, care planning, and ongoing evaluations.
- Monitor patient conditions and respond to changes in health status.
- Administer medications, treatments, and wound care as ordered.
- Perform clinical skills including venipuncture, catheterization, wound care, and parenteral therapy.
- Supervise Home Health Aides and CNAs to ensure quality patient care.
- Educate patients and families on disease management, medications, and home care.
- Maintain accurate documentation in compliance with OASIS and agency standards.
- Collaborate with physicians and interdisciplinary teams to ensure continuity of care.
- Participate in on-call rotation and quality improvement initiatives.
