Job Title: Hospice Liaison Profession: Operations Specialty: Home-Hospice Care Operations Duration: 13 weeks Shift: 10hr Day Shift Hours per Shift: 40 Experience: Minimum 2 years preferred in hospice liaison or other hospice experience License: Driver's License Certifications: BLS, Auto Insurance, Vehicle Must-Have: - EPIC experience required - Comfortable with end-of-life symptom management - Ability to educate providers on hospice benefits and philosophy - Understanding different levels of hospice care Description: The position involves liaising between hospitals and hospice services. The role includes monitoring patient referrals from hospitals or skilled nursing facilities for hospice care. Liaisons will provide assessments and recommendations for appropriate hospice settings. This position may require travel to multiple hospitals, assigned based on size and patient census. Common patient types include adults with heart failure, cancer, and other end-of-life diseases. Candidates must be comfortable collaborating with physicians and providing education on hospice services. The liaison team will be available for support and collaboration. Additional required skills include understanding the different levels of service hospice can provide. A minimum of two years of experience is required, with preferences for hospice liaison experience. The role consists of Monday to Friday shifts between 08:00 and 18:00, with rotating days off.
