Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 hours Experience: Minimum of 1 year of Certified Home Care experience, 1 year of Case Management experience License: RN License required Certifications: Relevant certifications in Home Health care preferred Must-Have: Experience within a certified home health agency only Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is required. Experience in hospice, private care, or working as a Case Manager without certified home health agency experience does not meet the qualifications. Candidates must be able to travel to assigned cases. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions and implementing care plans. Providing ongoing education and support to patients and their families is essential. Patient care will be delivered within established organizational philosophy, policy, and standards of community health practice. Performing admission processes and case management responsibilities in accordance with the organization's policies and standards of practice is required. Thorough and timely electronic documentation and interdisciplinary communications are essential elements of the role.
