Job Title: RN Home Health Profession: RN Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: Active Registered Nurse license Certifications: Certified Home Care certification required Must-Have: 1 year case management experience; ability to travel to assignments; weekend rotations every 6 weeks Description: All applicants must have 1 year of certified home care experience. 1 year of case management experience is required. Ability to travel to cases as assigned is necessary. Weekend rotations occur once every 6 weeks. Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Admission processes and case management responsibilities must be performed in accordance with established policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communications are required.
