Schedule: 40 Hours/Week (5x8s)

Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care for Home Health and Hospice patients.
- Provide skilled nursing services, including disease management, wound care, medication administration, injections, and IV therapy.
- Monitor patient progress, evaluate treatment outcomes, and update care plans as needed.
- Educate patients and family members regarding diagnoses, medications, treatments, and self-care management.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.
- Accurately document patient assessments, interventions, and outcomes in electronic medical records (EMR).
- Deliver compassionate end-of-life and palliative care support when applicable.
- Maintain compliance with agency policies, nursing standards, and regulatory requirements.
- Demonstrate flexibility and adaptability in response to changing patient needs and schedules.
- Travel to patient homes within the assigned territory while maintaining professional and patient-centered care.
