Must-Have:
- Proficiency in OASIS and experience with HomeCare HomeBase EMR within the last 6 months.
- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Description:
Provides coordinates and directs the provision of home nursing care according to physician orders.
This is based on agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.
Home visits are required along with periodic office meetings.
Must be willing to travel up to 75 miles from assigned care center.
Mileage will be reimbursed at the IRS standard rate.
Contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points.
The average number of patients per day is 5-8.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
All documentation must be completed within 24 hours of the appointment with the patient.
Delayed and incomplete documentation that impacts billing and follow-up patient care may result in a cancelation.
Expectations include hitting a weekly productivity point total of 30, which equates to 40 hours per week.
There is a rotation of on-call duties, which varies per location and profession.
Local candidates are accepted at the same rate.
Candidate must understand that there are no guaranteed hours with this contract.
If insufficient patients are received each week to support financial needs, the contractor may have the opportunity to float to another nearby clinic to pick up patients.
