Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty. License: Current nursing license required. Certifications: CPR, BLS Must-Have: Recent specialty experience required. Proficiency in HomeCare HomeBase and Oasis within the last 6 months required. Must have reliable transportation and current driver’s license. Must have agency-required liability insurance. Description: Provides, coordinates, and directs the provision of home nursing care. Care is provided according to physician orders based on agency policies and procedures. Competent application of the nursing process is essential. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits are required, with periodic office meetings. Must be willing to travel up to 75 miles from the home clinic. Average number of patients seen per day is 5-8. Contractors will be assigned between 5-10 patients per day. Documentation, travel, and visits hold different point values, with a total of 30 points expected each week. Types of cases include Start of Care, Wound Care, and Follow-up visits. Documentation must be completed within 24 hours of the appointment. It is expected that clinicians accept every patient scheduled. There are no guaranteed hours, and a minimum of 30 points must be met weekly. Any necessary floating to nearby clinics will be discussed as needed.
