Description:
As a Home Health RN, you will be responsible for assessing patient conditions.
You will implement care plans and provide ongoing education and support to patients and their families.
Patient care is delivered within the organizational philosophy and policies.
You will also perform admission processes and case management responsibilities according to established policies and standards of practice.
Utilizing thorough and timely electronic documentation and interdisciplinary communications is required.
Weekend rotations are once every 6 weeks.
Start date is based on credentialing clearance.
