Description:
Provides, coordinates, and directs the provision of home nursing care according to physician's orders.
Carries out responsibilities based on agency policies and procedures through the competent application of the nursing process.
Reports directly to the Clinical Manager.
Supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conducts home visits and attends periodic office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points based on visit types and activities.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be willing to travel up to 75 miles for patient care.
Mileage is reimbursed at the IRS standard rate.
No guaranteed hours; if unable to meet financial needs due to low patient census, the contractor may discuss options to float to nearby clinics.
Travelers must accept every patient scheduled regardless of distance from previous appointments.
All documentation must be completed within 24 hours of patient appointments.
Delayed or incomplete documentation that affects billing and patient care will result in cancellation.
Must be open to on-call rotations occurring every 3-4 weeks.
Home health experience is needed.
