Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 8x5 Days Hours per Shift: 8 Experience: - Minimum of two (2) years of experience in a home setting within the last ten (10) years - Recent experience conducting comprehensive assessments of patients is required License: - Current RN Licensure in Client Certifications: - Basic Life Support (BLS) is required before the start date Must-Have: - Two (2) years of experience in a home setting, which may include working in the field for a certified home health agency, licensed home care service agency, or managed long-term care - Experience conducting assessments similar to OASIS or UAS - Providing primary care to homebound individuals - Care management for individuals receiving home and community-based services - Experience as a paraprofessional or licensed practical nurse caring for a minimum of two patients per week Description: Required licensure includes current RN licensure in Client. A minimum of two years of experience in a home setting within the last ten years is required. Nurses must have recent experience conducting comprehensive assessments of patients. Candidates are expected to conduct hybrid telehealth and in-field assessments. Candidates must engage in continuous collaboration and communication with consumers regarding their health care options. The role involves working independently and effectively managing multiple tasks in a fast-paced environment. Excellent organization, interpersonal, and communication skills are essential.
