Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Shift: 5x8 Days Hours per Shift: 8 Experience: Two (2) years of experience in a home setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: Recent experience conducting comprehensive assessments of patients Description: This position requires the ability to review health status questions and make evaluations and determinations based on criteria for enrollment. Candidates must be comfortable working with technology in the workplace. Knowledge of the theory, principles, and practices of professional nursing is essential. Understanding of state and agency laws and regulations governing nursing practices is required. Strong organizational, interpersonal, written, and verbal communication skills are necessary. Candidates should demonstrate sensitivity to the concerns of others. The ability to perform comfortably in a fast-paced, deadline-oriented work environment is crucial. Candidates must successfully execute many complex tasks simultaneously. A strong ability to work independently is required. Responsibilities include: Performing initial in-home evaluations of patients to assess health status, strengths, care needs, and preferences. Reviewing consumer medical documentation and relevant health referral forms. Entering evaluation data into electronic forms and transmitting as required. Documenting concerns and issues that arise during the evaluation process. Emphasizing continuity of care to reduce fragmentation and gaps in treatment plans. Discussing healthcare options, needed supports, service vendor options, and waiver options with consumers. Facilitating communication with eligible consumers for enrollment into managed care health plans. Enhancing communication and collaboration with interdisciplinary care team members to improve care coordination. Collecting quality review data and necessary documentation to support outcome measurements. Identifying opportunities for health promotion and illness prevention. Maintaining a comprehensive working knowledge of community resources and payor requirements. Meeting all established standards for the position as outlined in annual performance criteria. Performing other duties as assigned by project management.
