Description:
Provide, coordinate, and direct the provision of home nursing care according to physician's orders based on agency policies and procedures.
Utilize the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Degree of travel involves home visits and periodic office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
If there is a situation where the contractor cannot meet their financial needs due to low patient census, please reach out to management to discuss floating to a nearby clinic to pick up hours.
Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
Candidates must have used HomeCare HomeBase within the last 6 months or be required to take a retraining course.
RNs are required to be proficient in OASIS.
The territory mileage radius requires traveling up to 75 miles.
Mileage will be reimbursed at the IRS standard rate.
Traveling professionals will be compensated from patient home to patient home.
Commute to and from the first and last patient are not billable.
Local candidates are accepted at the same rate.
Candidates with pending licenses will not be accepted; a current license is required.
Clinicians must understand that they will be responsible for driving to patients' homes daily and to the clinic for weekly meetings.
Candidates must have a vehicle, driver’s license, and auto insurance.
The mileage radius for the job is 50-75 miles.
Clinicians must be comfortable with potentially longer drives between patients.
Clinicians are expected to accept every patient on their schedule, regardless of distance.
All documentation must be completed within 24 hours of the appointment with the patient.
Delayed or incomplete documentation impacting billing and follow-up patient care may result in cancellation.
There are NO guaranteed hours with this contract.
Clinicians must fulfill weekly productivity points of 30 to equate to 40 hours per week.
All overtime must be pre-approved, and any insufficient patient load will allow the opportunity to float to another nearby clinic.
