Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health or Hospice experience License: Valid nursing license required Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and Oasis Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders. Applies agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Required to make home visits and attend periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day: 5-8. Contractors will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, Follow Up Visits, etc. Must be open to traveling up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Documentation must be completed within 24 hours of the patient appointment. All documentation must be timely and comply with requirements. There are no guaranteed hours with this contract. Clinicians must achieve weekly productivity points. Must be proficient in OASIS and HomeCare HomeBase EMR. Accepting patients is expected regardless of distance between appointments. All overtime must be pre-approved. Not open to first-time travelers; recent experience is required. Must be willing to participate in on-call rotations as needed.
