Description:
Provides coordination and direction for the provision of home nursing care according to physician's orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits are required, along with participation in periodic office meetings.
Reliable transportation, a current driver's license, and agency-required liability insurance are necessary.
Average number of patients seen per day ranges from 5 to 10.
Contractor will be assigned between 5-10 patients per day.
Different types of visits and activities will have varying point values.
No guaranteed hours; if a contractor is unable to meet financial needs due to low patient census, they can reach out to management for options.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Traveling up to 75 miles from the assigned care center is required.
Mileage will be reimbursed at the IRS Standard rate.
On-call duties may be required on a shared basis once a month.
Weekly productivity expectations include 30 points, which equates to approximately 40 hours of work.
Documentation must be completed within 24 hours of the appointment with the patient.
Delayed or incomplete documentation impacting billing and patient care will be addressed.
Clinicians must accept all patients on their schedule, regardless of distance from previous appointments.
This is a home health assignment with responsibility for driving to patient homes daily.
