Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty. License: Must have a current nursing license. Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase (HCHB) and Oasis. Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures through the competent application of the nursing process. Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Performs home visits and participates in periodic office meetings. Must have reliable transportation and a current driver’s license with agency-required liability insurance. Average number of patients is 5-8 per day. Points are accounted for each visit and activity. Contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must have recent specialty experience. Weekend requirements vary per location and profession. Open to on-call rotations, depending on specific location needs. Must be willing to travel up to 75 miles from the assigned care center for patient visits. Mileage reimbursement is provided per IRS Standard rate. Local candidates are accepted at the same rate. Must not be a direct employee of any health care location when applying as a traveler. Documentation must be completed within 24 hours of the appointment with the patient. Delayed or incomplete documentation may impact billing and follow-up patient care. There are no guaranteed hours with this contract. Weekly productivity points of 30 must be met, equating to approximately 40 hours per week. All overtime must be pre-approved. If weekly patient census is low, there are opportunities to float to another nearby clinic for hours.
